Membership Application

To apply to AMSA@FAU, you must first download and fill out this form. Supply the required information and take the completed form to the next AMSA pre-med meeting.

Shadow Agreement Form

This form states the guidelines for the shadowing program. Members should download this form if they intend to participate in any shadowing opportunities throughout the year.

Shadowing Participation Form

This form must also be completed with any intentions to shadow a physician. Submit the filled out form to the Director of Volunteering.

A comprehensive list of the duties expected of each AMSA@FAU officer. Please refer to this guide if you plan on being involved as an officer in the future. We are always looking for new leaders that will help AMSA achieve its goals.

Service-Learning Log

This form will allow you to keep track of volunteering hours accumlated throughout the year. Use this form to submit an official record of logged hours.

Campus Volunteer Center Participation Form

Any volunteering programs will require this form to be completed for official record-keeping. Once completed, submit this form to the Director of Volunteering.

Officer Election Application

Prospective candidates must turn in this form before election week. Make sure the position you intend on running for is clearly indicated and a short essay discussing your goals as an officer accompany the form.
